PESTICIDE RECORD KEEPING REQUIREMENTS

Private Applicator Restricted Use Pesticide Application RecordCertified private pesticide applicators must maintain records of federally restricted-use pesticides applications pursuant to the USDA requirements.  These records must be retained for a period of two (2) years following the application of the restricted-use pesticide. Items to be recorded are:

  1. The brand or product name of the federally restricted-use pesticide and the product’s EPA Registration number.

  2. The total amount of restricted-use pesticide applied.

  3. The size of the area treated with the restricted-use pesticide.

  4. The crop, commodity, stored product, or site treated with the restricted-use pesticide.

  5. The location of the application of the restricted-use pesticide.

  6. The month, day, and year of the application.

  7. The certified applicator’s name and certification number.

Professional Applicator Pesticide Application Record– Certified professional applicators must maintain records of all pesticide applications pursuant to IDAPA 02.03.03.150.  These records must be retained for a period of three (3) years following the application of the pesticide.  Items to be recorded are:

  1. The name and address of the owner or operator of each property treated; and

  2. The specific crop, animal, or property treated; and

  3. The location by the address, general legal description (township, range, and section) or latitude/longitude of the specific crop, animal, or property treated; and

  4. The size or amount of specific crop, animal, or property treated; and

  5. The trade name or brand name of the pesticide applied; and

  6. The total amount of pesticide applied; and

  7. The dilution applied or rate of application; and

  8. The EPA registration number of the pesticide applied; and

  9. The date of application; and

  10. The time of day when the pesticide is applied; and

  11. The approximate wind velocity; and

  12. The approximate wind direction; and

  13. The full name of the person recommending the pesticide application; and

  14. The full name of the professional applicator applying the pesticide; and

  15. The license number of the professional applicator applying the pesticide; and

  16. Worker protection information exchange, if required by the worker protection standard, prior to pesticide application, shall be documented by:

 

Date of contact; and

 

Time of contact; and

 

Name of grower or operator contacted.

Dealers – Pesticide dealers must maintain distribution records of restricted-use pesticides pursuant to IDAPA 02.03.03.150.03.  These records must be retained for three (3) years.  Items to be recorded are:

  1. The name and address of the person purchasing or receiving the restricted-use pesticide.

  2. The certified applicator name, license number and expiration date of the license for the person applying the restricted-use pesticide.

  3. The name, license number and expiration date of the pesticide dealer when a restricted-use pesticide is distributed to another dealer.

  4. The brand name and Environmental Protection Agency (EPA) Registration Number for the product distributed.

  5. The date of distribution of each restricted-use pesticide.

  6. The quantity and size of each restricted-use pesticide container and total quantity of each restricted-use pesticide distributed.

  7. The pesticide dealer’s name, address and dealer license number distributing the restricted-use pesticide.
